




Our Story
We recognize that all elders have a lived story that matters; they are not their disease or illness; they are a culmination of decades of lived experiences that we need to value and incorporate into any care-giving. We believe in taking a pause, slowing down, listening to understand the voice and vision of the senior versus trying "to fix", and "do for". We support working "with" the elder in their self-care management based on what matters most to them building on their strengths and lived experiences.
Our Organization
Empowers seniors to live independently in their home by leveraging community resources and existing health insurance, supportive partnerships, and volunteer professionals. It is an experience and place where an elder shares their lived-story and volunteer nurses, and Community Health Workers (CHWs) build upon that foundation to assist the elders’ self-care management to age well in their place.
Through a relationship-based approach, the care team honors the elders voice, providing free social, functional and self-care services in their community, bridging services to help close and mitigate care gaps, and enhancing community partnerships to best serve the elders self-care management daily needs. Our team of volunteers works with multi-cultural elders to honor and amplify their voice in self-care management.
Our 'Why'
... arises from our personal and professional lived experiences.
Personally, our founders know that caring or being cared for, begins with a listening heart. Caring for our own family members as they aged in place provided us with decades of deep personal experiences; we understand the complexities of navigating the healthcare ecosystem and changing roles. Our experiences with our aging elders who lived with chronic care needs, hearing, vision, and cognitive challenges that emerged for them as they aged in their home was not easy, and often we felt lost; all had good access to health care and caring clinical teams. The reality for our family... it was the in-between care visits, how to maintain post health system visit, or the social and functional aspects of aging well independently that required more attention at home.

Our Why....
Professionally, we have worked in public health, chronic care management, care coordination, clinical, and functional care delivery systems with numerous older adults who desire to "age in place"; we have deep experiences with culturally diverse, low-income and marginalized communities. We have clinical academic experience and have witnessed the immense relationship value between elders and emerging professionals.
